🛠 Introduction to the Tool

Apollo Root Cause Analysis (ARCA) is a cause-and-effect-based problem-solving method that focuses on finding systemic causes, rather than just symptoms. Unlike linear tools, ARCA embraces complexity, helping teams map out all contributing factors to a problem using an evidence-based logic tree. It is particularly valuable in project management and internal audit, where understanding interdependencies is key to sustainable corrective action.

The goal of Apollo RCA is not only to solve the problem, but to prevent recurrence by addressing the full chain of causality.

🧭 Template for Applying Apollo RCA

To apply the Apollo RCA method:

  • Step 1 – Define the Problem: Clearly describe the undesirable outcome.

  • Step 2 – Collect Evidence: Gather data, documents, interviews, logs, etc.

  • Step 3 – Build a Cause-and-Effect Chart: Connect conditions and actions that contributed to the outcome.

  • Step 4 – Identify Effective Solutions:

    • Remove causes

    • Modify conditions

    • Strengthen controls

  • Step 5 – Implement & Monitor: Assign owners and track implementation.

🏢 Example: ACME Corp – Project Delay on Digital Rollout

At ACME, Internal Auditor Auren was called to review a delayed digital rollout led by Project Manager Aven. CEO Liora requested a full review to avoid future overruns.

Using Apollo RCA:

  • Auren mapped the root cause tree: The delay (problem) was caused by delayed testing (action) + underestimated resource needs (condition).

  • Further back, resource underestimation stemmed from vague scoping, and testing delays were worsened by unclear task ownership.

  • Recommendations included improving scope definition workshops, assigning test leads early, and revising the resource estimation model.

Conclusion:
Apollo RCA helped Auren and the ACME team visualize interconnected causes and identify fixes that improved both planning and execution. This tool supports strong, logic-based decisions in complex project environments.
